- Disability Rating Letter
- Letter from medical or mental health provider stating you would benefit from having a service dog.
Once we receive all of this, the board will go through it, either approve or deny and will contact you by phone.

Upon completion of your training, you will receive a "Certificate of Completion" as there is no such thing s a "Certified Service Dog."
