EVERY PERSON INVOLVED IN ANY 

TRAINING MUST FILL OUT THESE FORMS!

You will need to email info@rmsdp.org and you will be assigned to an On-Boarding Team member who will reach out to you.  Please make sure you include your name, phone number, if you already have a dog and any other information you think we will need. Once you have spoken with them, they will send you the following documents. 

• New Client Questionnaire 

• Fundraising Agreement

• Service Agreement

These documents must be filled out and returned with a copy of the documents listed below.  

• DD214 (link if lost) https://www.archives.gov/veterans/military-service-records/about-service-records 

• Disability Rating Letter

• Letter from medical or mental health provider stating you would benefit from having a service dog.

Once we receive all of your documentation, the On-Boarding Team and either the President or Vice President will go over everything and make sure you are a good fit for our organization.  You will be notified by phone and email of your acceptance or denial.

If accepted, we will then set you up for an evaluation if you have your own dog, if not one of the trainers will reach out to you to help determine if we already have a dog that will fit your needs or if we need to look for one.  

Final stage will be your $500 payment (which will be explained during on-boarding), you will be issued a vest with patches, added to the WhatsApp training group and you will start attending training. 

RMSDP will provide the following;

1. Public Access Training

2. Task Training (2 Tasks)

3. ADA Education

4. Basic Obedience (Come, sit, down, heel and place.)

5. Service Dog Testing

6. Initial Service Dog Evaluation

Upon completion of your training, you will receive a "Certificate of Completion" as there is no such thing s a "Certified Service Dog."

Service Dog Tasks

Pick 2 tasks you would like your service dog to preform. Please remember not all dogs can preform all tasks. So please have an alternative if the task is something your dog won't be able to do.  This is just a short list of tasks, there are hundreds of tasks we just wanted you to see a few. 

1. Alert to People Walking Up from Behind.

2. Alerting Family Members.

3. Alerting for Help.

4. Answer the Door.

5. Assist with Grounding.

6. Assist with Transfers.

7. Assisting with Position Changes (e.g. Sitting to Standing, Laying to Sitting, etc).

8. Block approaching persons/animals, etc.

9. Call 911 on a Dog-Friendly Phone.

10. Call Suicide Hotline on a Dog-Friendly Phone.

11. Closing Doors (pulling closed with a tug).

12. Closing Doors (pushing closed with nose or paws).

13. Counter-Balance.

14. Crowd Control (circling).

15. Deep Pressure Therapy (DPT)

16. Find a Bathroom.

17. Find a Specific Person.

18. Find the Car.

19. Forward Momentum (in a wheelchair).

20. Forward Momentum (when walking).

21. Go Find Help.

22. Guide to an Exit.

23. Interrupt Crying, Dissociating, and Freezing Behaviors.
